A balanced 360 feedback template uses a small set of role-relevant competencies, behavior-based statements, a consistent rating scale, and open questions about what to continue, change, or begin. Aggregate patterns instead of overreacting to one comment, then translate the results into one or two observable development actions.

Structure the 360 feedback survey

Choose a small set of competencies connected to the person’s role, such as communication, collaboration, decision-making, leadership, and execution. Write statements about observable behavior and use a clearly defined rating scale. Add open questions about strengths, impact, and one behavior that would improve effectiveness.

Interpret the results responsibly

Protect anonymity, explain how raters are selected, and avoid presenting an average score without context. Look for patterns across rater groups and comments. In the debrief, identify a limited number of development priorities, define what improvement would look like, and schedule a later check-in to review progress.
